Fairplex pays 90% of HMO premiums and 85% of of our PPO premiums for our team members.

Dental Insurance:
Fairplex pays 90% of DMO premiums and 85% of the DPO premiums for our team members.
Vision Insurance:
Select from the network provider or your own opthamologist/optometrist and Fairplex pays 90% of the premium for our team members.
Cafateria Plan:
Allows our team members to set aside pre-tax dollars for qualified health expenses, non-covered health expenses or qualified dependent care expenses.
Life Insurance, Accidental Death & Dismemberment (AD&D):
Life insurance provides financial protection for your loved ones in case of your death. AD&D coverage offers added protection if an accident causes loss of life, limbs and/or senses.
Voluntary Coverage:
Voluntary Life Insurance for yourself is available in $10,000 increments to a max of 5 x annual earnings; or $500,000 and is also available for your spouse in $5,000 increments to a max of $250,000.
Child(ren) Voluntary Life is available in the amount of $1,000 for each child between birth to 6 mos. Coverage for 6 mos. to age 19 (to age 26 if FT student) is available in $2,000 increments to a maximum of $10,000.
Short-Term Disability (STD):
Basic STD coverage that will replace 60% of your monthly salary, up to a maximum benefit of $1,155 per week, at no cost to you. STD benefits begin on the 30th day of disability from injuries and illness, for a maximum of 9 weeks. At that point, your LTD benefits will begin, should your disability continue. Partial benefit is also available for those who are able to work on a part-time basis.
Long-Term Disability (LTD):
Basic LTD coverage that will replace 60% of your monthly salary, up to a maximum benefit of $2,500 per month, at no cost to you. LTD benefit begins after you have been disabled for 90 days and continues until your SSNRA, as long a you remain disabled. LTD plan benefits are reduced by the amount of any state disability, worker's comp and SSI income you may receive.
